Understand the truth about sciatica and how to find effective relief.
Many people who come to see me think they have sciatica pain because they experience pain
radiating from the lower back down the legs. However, sciatica is more specific than you
might think.
What is Sciatica?
Sciatica is a condition characterised by pain radiating along the course of the sciatic nerve.
This nerve, the largest in the body, originates from the spinal nerves L4 to S3, running from
the lower back through the pelvis and down the legs, controlling movement and sensation in
the lower limbs.
Despite being one of the most common causes of pain radiating from the lower back to the
legs, sciatica is often misunderstood. Importantly, sciatica is not a disease but a symptom of
underlying nerve compression or irritation. A proper diagnosis is crucial to differentiate
sciatica from other conditions and to determine the most effective treatment for your
symptoms.
Diagnostic Criteria:
Sciatica is diagnosed through clinical evaluation, including physical examination, patient
history, and sometimes imaging (MRI or CT scans) to confirm nerve compression or other
underlying causes.
If you're struggling with persistent discomfort or unsure whether your symptoms are truly
sciatica, these five insights will help clarify the condition and guide you toward effective
relief.
1. Not All Lower Back and Leg Pain is Sciatica
Not all pain in the lower back or legs stems from sciatica. True sciatica occurs when the
sciatic nerve is compressed, often due to conditions like herniated discs, spinal stenosis, or
bone spurs. Other conditions, such as Piriformis Syndrome or muscle strains, can mimic
sciatica but require different treatments.
Tip: A proper diagnosis from an experienced physiotherapist is essential to ensure the right
care for your symptoms.
2. Sciatica Symptoms Can Vary Widely
Sciatica doesn’t always present as sharp, shooting pain. Symptoms can also include:
Tingling or numbness in the buttocks, legs, or feet.
Weakness in the affected leg, making walking difficult in severe cases.
Pain that worsens with sitting, coughing, or sneezing.
Understanding your specific symptoms is key to tailoring an effective management and
treatment plan.
3. Rest Isn’t Always the Best Solution
While taking it easy for a day or two might provide short-term relief, prolonged rest can
worsen sciatica symptoms. Movement is crucial for recovery. Gentle exercises prescribed by
a physiotherapist can alleviate nerve compression, improve flexibility, and strengthen
muscles supporting the spine.
Did You Know? Staying active with physiotherapist-recommended stretches and low-impact
exercises can accelerate recovery.
4. Sciatica Can Often Be Treated Without Surgery
Surgery is typically a last resort for severe cases of sciatica that don't respond to conservative
treatments. Most people find significant relief through non-surgical options, including:
Physiotherapy.
Targeted stretching and strengthening exercises.
Manual therapy to reduce nerve irritation.
Dry needling and soft tissue mobilisation.
Our experienced physiotherapists provide accurate assessments and personalised treatment
plans to address the root cause of your sciatica symptoms.
5. Preventing Sciatica is Possible
Once you’ve found relief, preventing recurrence is essential. Here are some tips to keep
sciatica at bay:
Practice good posture, especially during prolonged sitting or standing.
Use ergonomic furniture to support your lower back.
Stay active with regular exercise such as walking, running, cycling and swimming.
Avoid heavy lifting or improper lifting techniques.
Regular physiotherapy progress check-in can help identify and address potential triggers
before they become issues.
